Due to my recent comments on model release forms, I've gotten a lot of requests for copies of the aforesaid forms. SO... rather than spend the rest of my life cutting and pasting from MSN Word, here it is -- the General Release form:

people should write their own model release forms, only the photographer knows whats best and what their work will be used for, better to be as precise and specific to your work than to use a generic form letter.......
this is a great basis for building your own personal model release.
I mostly use this form for clients who are worried about nude photos appearing on the internet. I just write "no internet" in the space marked authorized usage.
MODEL LIMITED CONSENT AND RELEASE
(A) The undersigned model and photographer hereby agree to accept all conditions set forth in this limited consent and release.
I, the undersigned model, hereby give the undersigned photographer, his clients and assigns, specific permission to publish, copyright, distribute and/or display photographic images of me taken on the below listed date. The permission granted herein is strictly limited to the uses described below and for no other purposes, whether suggested or implied.
I, the undersigned photographer, agree that the model has the right to inspect and/or examine all photographs and/or written text to which the images may be applied before use as described below.
I, the undersigned model, hereby release and discharge the undersigned photographer, his clients and/or assigns from any and all liability by virtue of distortion, blurring, alteration, optical illusion, and/or use in composite form, whether the same is intentional or otherwise. I understand that the photographer, his clients or assigns may use any process or procedure resulting in the completion of the finished product, the publication, distribution or public display, when photos of me are used in accordance with the usage listed below.

I kind of agree, but I put a lot of thought into this and it covers every stiuation I can think of. This is what I use for paid models, and it is a blanket release. I also use a limited release though, which follows. I use that one mainly for clients (the people who pay ME). They are often worried about their naked selves appearing on internet porn sites. For those, there is a space titled "Authorized usage" where I can write "No internet" and any other restrictions.

Something I give to all prospective figure models. I meet them before the shoot and go over everything beforehand. I don't want there to be any surprises during the shoot. Eases the tension a lot if they know what is going to happen.
FIGURE MODELS
What You Can Expect
Paid figure modeling is nude or (very occasionally) topless. Clothed models can be had merely for asking. Pay is at the approximate rate of $50 per hour and does not include 30-minute breaks, but does include 15-minute breaks. In a two-hour photo shoot you will be in front of the camera for approximately an hour and a half (you’ll get two 15-minute breaks) and will be paid $100. During a four-hour photo shoot you will be modeling for three hours and will be paid $150. These rates and times are approximate and you will be paid $100 for a minimum of 10 rolls of film. If it takes longer, then…well…it will take longer.
1. You will be asked for proof of age. I do not photograph models under 18 years of age. There are no exceptions to this rule. Please remember to bring a photo-ID.
2. You have the right to refuse any pose. Please remember that you are being paid to do nude figure modeling however. Full frontal nudity is to be expected. If you wish, I will show you examples of my photography before the shoot. I don’t do pornography. When I feel that a pose is overly explicit I use darkroom effects to minimize its sexual impact. As a rule, I use poses that are relatively mild. You will be treated with respect at all times and I will assume responsibility for your safety. I do not touch my models unless there is no other reasonable alternative (a shoot involving body painting, for example), and then only with permission.
3. Please wear loose non-constricting clothing to your shoot. Leave your undergarments at home or pack them to change into after the shoot. Anything with elastic in it marks the skin and the shoot will have to be delayed until the marks fade. This would mean sitting around nude and unpaid for some time, so don’t wear elastic. You may wish to bring a bathrobe to wear during breaks or I can provide a blanket.
4. Before the shoot you will be given the opportunity to ask questions. There are no stupid questions; if you have any, ask them. The model- release contract will be explained to you. Again, if you have any questions, please ask them.
5. Photo shoots are conducted as follows (per 2 hour segment): I will shoot for half an hour. You will then be given a 15-minute break. Smoking is permitted. I will then shoot for another half hour. At this point, should you desire one, you will be given a 30-minute break. After another half hour you will get another 15-minute break. Finally I will shoot for another half hour and you may get dressed.
6. After the photo shoot you will be paid and will be asked to sign a release. Signing the release is a condition of being paid. The photos are useless without a signed release.
7. The last step is to fill out one of my model’s feedback forms. This will help me to make my models more comfortable in future photography sessions. My aim, aside from making beautiful photographs, is to make my models as comfortable as possible and to make this an enjoyable experience.
8. If at any point in the future you should regret your decision to model for me, you may purchase all of the negatives and prints of your photo session by refunding the modeling fee and the cost of the materials. All negatives and prints of your photo shoot that are in my possession will then be destroyed.
9. You may bring a friend if you wish, but no boyfriends and not your husband. They invariably, in my experience, want pornographic photos and we would both find that embarrassing and distasteful. I would like to reiterate that you will, in any event, be quite safe and you will be treated with respect.
10. Up to five selected 8 x 10 “straight” photography prints of photos taken during your modeling session may be purchased at cost. Alternatively, two special effects prints made from photos taken during your photography session may be purchased at cost. Additional prints will be priced at $50 each for straight photography or $100 each for special effects photography. These prices are for models only.
11. Modeling in exchange for prints may be negotiated but I will retain the negatives and the copyright.

There is a book of legal forms for photographers that comes with a CD of the forms that is similar to those already given but it has others also. Another book called the law in plain english for photographers to good also on copyright and such.....both books would be good for personal usage...
