What it says... I often find myself needing this and not being able to use it. How come we're allowed question marks and not exclamation marks? They're equally important at getting emotion across in a title.
Might seem a silly suggestion but it really bugs me sometimes.

A relatively common suggestion. I have some titles that would have used this character. Or French accents, for names. Might be an old programming limitation, as suggested. Now they replace special chars by underscore _ in filenames and URLs. Or it is to avoid excessive titles, like "Super!!!1!!!one!!!!" Perhaps they will lift this limitation when they will switch to UTF-8.

I don't think the potential overuse of exclamation marks should stop the less overexcitable artists ( ) from using them though. If anything, the use of exclamation marks might in some odd way make spam easier to find, as spam artists may be more likely to title their pictures with "omg!!!!111!!"?

If my title wont fit or if I have alternate titles in mind or (like you said) the title has special punctuation, I usually put a note in my artist's comments about it. Something along the lines of "Actual Title: blahblahblah" or "Full Title-"
Most people don't read Artist's Comments, but it's still better than nothing.
