The Proposal? Or not to have one

I've heard about sending in a Query letter to an agent or editor is a way to get their attention.

Then, I've read about how one needs a Proposal, with outlines, research, costs that a writer will use with the advance to make the book and more. There's books on publishing,and making proposals.

So, what exactly should a writer send in? The whole she-bang with a several paged Proposal with a cover letter, bio and more? Or just Query them first, and send a few sample chapters.

What works best?

Some just query, others do the whole Proposal. What if your work is done?
