How to draw themes or icons for windows? (Photoshop)

I wanna know how would you create an icon for a program on Photoshop, how should I save it? Would it work the same on windows XP and 7?
For themes... am just curious....
PS: I use Win XP but I will change to 7 soon.

i use only window paint to draw pixel icon
save it as gif file
if you want to animate it, use "adope imageready" instead of "adope photoshop"
imageready is perfect for animated gif icon

Icons, for folders or .exe files, not avatar icons... or can you animate those as well?

- Photoshop/Illustrator/GIMP
- Save your stuff as PNGs
- Save your icons in multiple sizes (16x16, 24x24, 32x32, 48x48 and 64x64)
